| Kindergarten and New Student Registration |
KINDERGARTEN and NEW STUDENTS 1st – 5th GRADE
Kindergarten Registration will be held on Thursday, March 29, 2012 between the hours of 8:00 a.m. to 12:30 p.m. in the Auditorium.

- Your child must be 5 years old on or before November 1, 2012.

- Registration packets will be available at the front office starting March 19, 2012.

- Please completely fill out the paperwork in the registration packet and have the following documents ready for copying the day of registration:
1.
Official Birth Certificate, Passport or Baptismal Certificate
2.
TWO (2) Proofs of Residence which can be any of the following:

Mortgage Documents (Mortgage Statement, Quarterly Tax Bill)

Rental Agreement (Current – Signed by Landlord and Tenant)
-
Current Utility Bill (Gas & Electric, Telephone, Cable)
-
Military Housing Orders
3.
Immunization Record (signed by the physician).
Immunization requirements are as follows:
1.
Polio (OPV and/or IPV), 4 doses (3 doses meet the requirement if at least one
was given on or after the 4th birthday)
2.
DTP, 5 doses (4 doses meet the requirement if at least one was given on or
after the 4th birthday)
3.
MMR, 2 doses (both on or after the 1st birthday)
4.
Hepatitis B, 3 doses
5.
Chickenpox (1 dose if 12 and under) or the date of the disease
FIRST THROUGH FIFTH GRADE - NEW TO OUR SCHOOL
Fall registration for new (new to our school) students First Grade through Fifth Grade will also be accepted on Thursday, March 29, 2012. The same requirements apply and registration packets will be available at the front office.

- FIRST GRADE – Your child must be 6 years old on or before November 1, 2012.
- - WE WILL NEED ALL OF THE DOCUMENTATION MENTIONED ABOVE BEFORE WE CAN PROCESS YOUR STUDENT’S REGISTRATION.
